Educate yourself on all available insurance options
before choosing a plan for the year.
The practice accepts most major insurance plans. Please check with
your insurance company regarding your coverage options. It os your
duty to know what is or is not covered under your plan. Please review
your insurance booklet. We require payment of Co-Pays and deductibles at the time of
the visit. If you need to know how much will be due at the visit,
please call us a day in advance so we can check with your insurance
company. The amount we are provided by your isnurance company is never a guaranteed amount due
by you. This is just how all insurance companies operate. You may
actually owe more or less when the final payment is made by your
insurance. All Co-Pays and Deductibles are preset by your insurance
company and the contract you have signed with them. We do not control
how much your insurance pays or does not pay, or how much you owe. Charges not covered by
your insurance company are your responsibility.

CareCredit®
as a payment plan option.
